Medical Treatment Authorization and Liability Release
I am willing and able to participate in recreation and related activities at First Assembly Church-Memphis. In order that I may receive the necessary medical treatment in the event that I sustain injury or illness during participation in this activity, I hereby authorize the instructors or supervising staff to obtain medical treatment for me during the activity, and hereby hold First Assembly Church-Memphis, and its representatives harmless in the exercise of authority. I understand that this activity involves risk to the participant. I further acknowledge and understand that due to the nature of recreational sports, which involves physical activity, there is a possibility that I may sustain physical injury (minimal, serious, or catastrophic), in connection with participation. I further acknowledge and understand that I assume the risk of such physical injury by participation, and I further release the recreation department at First Assembly Church-Memphis, and its representatives from any claim for personal illness or injury that I may sustain during participation in the activity.
I understand that First Assembly Church-Memphis recreation department, from time to time produces promotional material relating to its programs. I understand that as a participant and/or spectator at an event I may be included in videotapes or photographs taken during the event. Therefore, without reservation or limitations I, hereby assign, transfer and grant to the aforementioned entities, its successors, assignees, licensees, and sponsors, the exclusive right to photograph and/or videotape me and to utilize such videotapes and photographs of me, face likeness, voice and appearance as a part of the event, in advertising and promoting First Assembly Church-Memphis, recreation department, or in advertising and promoting similar future events. I further understand that First Assembly Church-Memphis, recreation department, nor any third party is under any obligation to exercise any of the foregoing rights, licenses and privileges.
I further understand that the recreation department at First Assembly Church-Memphis has established rules and regulations pertaining to conduct, behavior, and activities of all participants, by which I must abide during participation in this activity, and that I will be responsible for failure to abide by those rules and regulations.
